Participant's preferred town(s), household preferences (living alone or shared), and any timing — current housing ending, hospital discharge, etc.
Funding position
Whether SDA is approved in the plan (and the design category if known), being pursued, or SIL-only / non-SDA housing is what is needed. Not sure? Send the referral anyway — we can help work it out.
